First Time  Passport Applicants and All Other Passport Renewals 

Passports for Children 16 and Older (New and Renewals), First Time Adult Applicants and Replacement Passports

Applicant(s) must be present at the time of application.

When you come to the Embassy to make your application, you must submit the following:

  1. A fully completed DS-11 passport application form. The DS-11 passport application form can also be filled out on-line. Press here for the on-line DS-11.
  2. Two recent, identical photographs, approximately two-by-two inches (5X5 cm). The pictures must be in color and should show the full frontal view on a light background. The Embassy will reject any photographs that do not meet the specified requirements listed on the application. Vending machine photographs and photographs printed on a home computer are unacceptable; 
  3. Original birth certificate (if born outside the United States the original Consular Report of Birth Abroad or a Naturalization Certificate is required).
  4. Most recently issued passport if applicable.  If unavailable due to loss, theft or if damaged, a DSP-64 must be completed and submitted with the application.
  5. 100 USD or the Icelandic Kronur equivalent for applicants 16 and older, payable in cash or with a credit card. The Embassy does not accept personal checks or debit cards.
  6. Proof of identity. The most common proof of identity is a driver's license.
