First Time Passport Applicants and All Other Passport Applications

Passports for Children 16 and Younger (New and Renewals)

Minor(s) must be present at the time of application.

When you come to the Embassy to make your application, you must submit the following:

  1. A fully completed DS-11 passport application form. The DS-11 passport application form can also be billed out on-line. Previous U.S. passport (if applicable)
  2. 1 photograph 5x5 cm (approx. 2x2 in.) with a white background, front view and a full face.
  3. Original birth certificate (if born outside the United States the original Consular Report of Birth Abroad is required or a Naturalization Certificate for first time applicants).
  4. Both parents must appear in person, with identification, to sign the application in front of the Consular Officer.  If one parent is unable to appear in person a notarized letter or a notarized form DS-3053 must be submitted with the application explaining the reason for the absence and giving permission to the parent present to apply for the passport.*
  5. Where there is only one parent, the signature of one parent is acceptable in cases where the parent can provide evidence of sole custody of the child.
  6. 85 USD or the Icelandic Kronur equivalent payable in cash or with a credit card.  The Embassy does not accept personal checks.
